Application Instructions
The following information is provided to help the prospective student successfully apply for admission to the Doctorate of Physical Therapy (DPT) program at the University of New Mexico. Although Graduate Studies strives to keep this information up-to-date, we still encourage you to visit the program's website to verify application instructions and required materials.
International students (including non-U.S. citizens on a visa) generally must complete additional steps to apply to a graduate program at UNM. Please see the Global Education Office's International Admissions website for more information.

| Fall Admission | The Graduate Studies PT Program application is how students who have been accepted through the PT Program’s application process are enrolled in UNM. Do not apply if you have not already been accepted to the UNM Division of Physical Therapy. Applications are due June 30th for students who were selected to join the program by the admission committee after applying through PTCAS. If you are unsure if you should fill out this application please contact Dr. Jodi Schilz (jschilz@salud.unm.edu). |

| Educational Background | List all schools attended and submit official transcripts. If you are graduating in May, wait for your degree to be confirmed/awarded (usually occurs 2-3 weeks after graduation) then submit your application. Students who attended UNM do not need to submit official transcripts for the time they studied at UNM, if you took courses anywhere else you must submit a transcript for those courses. If you have outstanding prerequisite classes you will submit an unofficial transcript to Dr. Schilz confirming course completion. |

| Special Requirements | You must have applied to the UNM PT Program in October through PTCAS, been interviewed in January, and accepted into the program before filling out this application.
